To install one (1) double-faced high density urethane foam sign to read tervis per drawings submitted on 08/31/15 & stamped approved 09/01/15 sign will measure: 36 w x 14 h x 3 thickness sign will hang perpendicular to the street, on existing bracket sign will hang from eye bolts sign will have a white background with multi-colored lettering no additional illumination or lighting is approved

Replace existing sign bracket with new 24" long, 1/2" diameter light fixture extension arm with adjustable 3" diameter light fixture, each lamp to have watt par lamp with lumens of 5 per lamp, per application and materials recieved 1/06/16 and stamped vcc approved 1/14/16.

To make the following modifications per vcc approval granted 02/19/13 & drawings received 01/04/13 & stamped vcc approved 02/27/13 for a change of use of the ground floor from retail to residential parking: modify the existing doors on the riverside ground floor faade so that the three leaves will operate as a single-leaf, in-swinging garage door re-enforce the existing threshold for the impacted ground floor opening replace sidewalk flagstones as necessary to match existing conditions install a curb cutnotes: the unauthorized sign must be addressed by either its removal or application made for its retention. The requisite permits must be received from the department of public works before work can proceed
