Interior alterations in the second floor only per plans approved by the philadelphia historical commission (no changes to exterior, no alterations to windows or doors; no work in third floor). Certificate of occupancy with no change in occupancy classification in second and third floors. Separate permits required any mechanical, electrical or plumbing work and for certificate of occupancy at fourth floor.

Interior alterations to third floor to create two new apartment units. Infill floor areas at old elevator shaft location, floors 2 thru 4. No other work allowed under this permit. Building is to be fully sprinklered and a certificate of occupancy obtained from the department of licenses and inspections prior to any occupancy above the first floor. Separate elect., plumbing, hvac and fire suppression permits required.

Replacement of flat rubber roofing in kind. No work to cornice or change in appearance. Work to be performed as per philadelphia historical commission approval.

Relocate 3 light fixture install 5 receptacles from existing circuit per 2008 ne (commercial)

1 fire panel, 9 horn strobes 2 smoke and 5 pull stations per 2008 nec &nfpa 72(office space)

Make safe permit - for (remove falling brick structure at rear of building as per engineer? S report if applicable) to resolve case #585032. Abutting sidewalk must be closed with fencing a minimum of 6? In height. Separate streets department permit required for sidewalk closure. A separate permit is required for any additional alterations that are not specifically addressed on case #585032.

Philadelphia historical commission registered property. Amend permit #638238 for replacement of deteriorated rafters and sheathing as required in addition to the installation of flat rubber roofing in kind. No work to cornice or change in appearance.
