To amend a p no 703722 to change to use of the second floor from storage to commercial kitchen, food preparation, storage, office and employee bathroom; for use by the restaurant on the first floor. Per proviso, the floor area with the reduced ceiling height shall be limited to the kitchen and back-of-house uses. Bbs case no 28823.

Erection of a four story addition to an existing detached structure (to be an attached structure) maximum height ne 38 ft with pilot-house roof access only (no roof deck). Size and location as shown on the application. Uses- eat-in restaurant and vacant commercial spaces #a & b on first floor (must occupy 100% of the ground floor frontage with a minimum depth of 30) and vacant commercial space # c on second floor(uses only permitted in cmx-2 that use not to require off street parking as per table 14-802-2, use registration permit is required prior to occupancy) and household living muti-family nineteen (19) dwelling units and eight (8) accessory bicycle parking on an accessible route.

To amend permit# 703722 to add non-bearing framing to the third floor level. Occupancy of this floor is not permitted as this time. Separate permit required for certificate of occupancy. Structure is fully sprinklered.

Asbestos abatement required prior to the commencement of any work for the erection of a three four story addition on the side and rear of an exiting detached building making it into an attached building. Building for use as two (2) vacant commericial spaces (separate use permit and fitout required prior to occupancy) with ninteen residential units throughout with storage and utilities on the basement level. Building to be sprinklered in accordance with nfpa 13 13r. All work to be done per approved plans. If field conditions vary contact structural engineer prior to the start of any work. Level iii alterations to include new doors and windows, framing, infill of openings and erection of partitions plumbing fixtures. Structure to be fully sprinklered. Separate permit required for certificate of occupancy, plumbing, mechanical, electrical and fire suppression.

For use of sit-down restaurant on the first and second floors, with an existing single-family household living unit above, within an existing detached structure. No change in height or area of existing building. No signs on this permit.
