On 3 16 2016, the department of licenses and inspections issued a notice of refusal for application #657630.

For the uses of two(2) sit down restaurants 1st floor and 2 vacant retail spaces(uses as permitted in district - separate use permits required) 1st floor, artisan industrials and artist studios 2nd - 7th floors(no residential uses). No signs this application.

First floor: unit 101 retail, unit 102 sit down restaurant, unit 103 sit down restaurant, 4th floor: unit 401 business and professional office, unit 403 business and professional office, unit 406 business and professional office, unit 501 atisan industrial

Interior and exterior alterations existing (7) story building to include basement. Interior demo, structural modifications, slab and rehab work, framing and support framing, partitions, shaft, new elevator cab in existing shaft, and new stair tower, framing and enclosure, vacant shell space commercial 1st floor, new fit outs studio units 2nd - 7th floors. Exterior to include new windows, awnings, new marquee, painting and patching brick, new accessible exterior ramp. Interior alterations to include new elevator car and machinery in existing repaired elevator shaft, repair work to existing stair tower and landings as per plans see bbs variance 27414 additional permits req: electrical, plumbing, hvac, sprinklers, signs. Co use permits and fit outs required for vacant commercial 1st floor spaces prior to occupancy.

For the installation of an automatic sprinkler system in accordance with nfpa 13 throughout an existing seven (7) story, mixed-used structure as per plans; using existing 6" fire line, new 6" backflow prevention device, two (2) new 4" class i standpipes within exit stairways installed as per nfpa 14, new fire pump in basement installed as per nfpa 20, and new fire department connections.

Amend building permit#6576 to include debris netting at north and south facades in accordance with the manufacturers specifications and as per approved plans.

Amend permit 6576 to convert 4th floor to three (3) business and professional offices (units 401, 403 & 405) and modify layouts of artist & artisan industrial studios on 5th floor as per approved plans. Building to be fully sprinklered.

Amend alterations permit no. 6576 to include accessible ramp, landing stairs and awning on n 10th street side of building as per approved plans.

For the lot adjustment to create one (1) zoning lot from two (2) existing zoning lots (10 and 1002-1004 buttonwood street)(three (3) deeded lots, two (2) opa accounts), size and location as shown on the plan.
